Third-Party Passenger Waiver and Acknowledgement

This waiver is for individuals who are not clients of Every Human Disability Services (“Every Human”) but who request to travel in a vehicle operated by Every Human as a guest or external support to a participant.

• I acknowledge that I am not a client of Every Human, do not have a Service Agreement or any contractual arrangement with Every Human.

• I acknowledge that Every Human will only bill its client for transport. I am not a client of Every Human and will not be billed, nor considered to be receiving a service.

• I agree to travel voluntarily as a passenger only, understand that my participation is limited to transport and do so at my own risk.

Every Human commits to ensuring that:

• The vehicle provided is roadworthy, registered, and appropriately maintained.

• The vehicle will be operated safely and lawfully at all times.

• The driver of the vehicle will:

  • Hold a current and valid driver’s licence suitable for the class of vehicle being used.

  • Be fit to drive (not under the influence of alcohol, drugs, or any condition that impairs safe driving).

  • Follow road rules and drive with due care for all passengers.

• I acknowledge that Every Human is not responsible for my personal duty of care, health, or wellbeing.

• I acknowledge that I am responsible for my own personal wellbeing, health, and needs during transport, and that if I require such support, I have the option to request and enter into an agreement with Every Human for this.

• If I am accompanied by my own support worker, or if I am an external support worker accompanying a client, I acknowledge that the external support worker or their employer remains fully responsible for providing the necessary care and support during transport.

• I accept that Every Human, its staff, directors, and representatives are not liable for any injury, loss, or damage arising from my travel as a passenger, unless such injury, loss, or damage results directly from Every Human failing to meet its responsibilities set out above.

• This waiver does not exclude liability where such exclusion is not permitted by Australian law
  

By signing this waiver, I confirm that I have read and understood the terms of this acknowledgement and agree to abide by them.
